Our vision is to enable people in the Clackmannanshire and Stirling Partnership area to live full and positive lives within supportive communities.
Integration of health and social care is the Scottish Government’s programme of reform to improve services for people who use adult health and social care services. It ensures that health and social care provision across Scotland are joined-up and seamless, especially for people with longer term and often complex needs, many of whom are older or disabled.
This makes it easier and quicker for you to access the services and support you require and help you to live safely and independently in your own home for as long as possible.
You won’t need to do anything differently – services have been working together behind the scenes. Health and social care staff are working more closely to support individuals and deliver joined-up care in local communities across Forth Valley.
Carers – Your journey, your support
As an unpaid carer to a relative or friend, your caring journey is unique to you.
We have developed a Carers Support Pack that details lots of organisations and groups that can support you as an unpaid carer. The Carers Support Pack can be found here.
