Our vision is to enable people in the Clackmannanshire and Stirling Partnership area to live full and positive lives within supportive communities.
Integration of health and social care is the Scottish Government’s programme of reform to improve services for people who use adult health and social care services. It ensures that health and social care provision across Scotland are joined-up and seamless, especially for people with longer term and often complex needs, many of whom are older or disabled.
This makes it easier and quicker for you to access the services and support you require and help you to live safely and independently in your own home for as long as possible.
You won’t need to do anything differently – services have been working together behind the scenes. Health and social care staff are working more closely to support individuals and deliver joined-up care in local communities across Forth Valley.
Transition from Children’s Services to Adult Health and Social Care Services
Young people with additional support needs hope for the same things as other young people; to be independent, to have a voice and a social life. It is important that the transition from one service to another is as seamless as possible. This can help ensure the young people of Clackmannanshire and Stirling have positive experiences and independence as they reach a new stage in their lives.
We would really like to hear your views on your experience, whether you are a young person that has already moved to Adult services or if you are a young person who will soon be leaving school.
We are also looking for the views of parents and carers of young people who have left school or who will be leaving school in the future.
