Needs Led, Resource Bound
The partnership are committed to delivering a programme of service transformation and modernisation programme to improve outcomes for service users and meet the challenges of increasing demand within significant financial constraints.
The 2022/33 Strategic Commissioning Plan sets out the priorities for the coming years and we need to deliver against these priorities through a ‘Needs Led Resource Bound’ approach. We require, through the programme to deliver over £10m of savings over the course of the year.
The key elements of the programme for 2024/25 are:
- Eliminating Reliance on Unfunded Service Provision
- Moving Our ‘front door’ social work services to a Right Care Right Time approach
- Implementing a Systematic and Singular Approach to Self Directed Support
- Putting in Place a Primary Care Medicines Optimisation Programme
- Charging for Care Services in Every Instance where Chargeable Care is Provided
- Tighter Controls and Management of Projects and Services
A Transformation Progress Review Meeting is held each month to provide oversight and monitoring of the programme and after each meeting a short briefing note will be published here:
